Please call our office in advance for an appointment if you are planning to attend in person to lodge your passport application.

The following instructions should be read in conjunction with those on the back of the application form before you attempt to complete the form.

Important Information
You must complete sections 1, 3, 4 and 5 of the application form. The completed form should be returned to our office together with:
• the passport you are replacing
• two photographs
• a photocopy of an identity document showing photograph and signature (For example: driver’s licence or Italian identity card), and
• payment in the form of “assegno circolare” made payable to the “Australian Embassy”.

Current application fees and information about additional postage costs is provided below.

Please ensure that:
 You use BLACK pen only
 You write within the boxes in BLOCK LETTERS
 You include the Australian Passport you are replacing with your Renewal Form. The passport will be cancelled and returned to you with your new passport.
 You Sign only within the white box in section 5. If your signature extends beyond the white box we may have to return the application and have you provide additional signatures.
 The PHOTOGRAPHS you provide meet the requirements detailed on the reverse side of the application form. If the photographs do not meet these requirements the application will be returned to you and you will be required to produce new photographs.

Seniors (optional for those aged 75 years or older).

The passport is valid for 5 years.    - Ordinary                           € 94

These fees are subject to change without notice. Please call and confirm the application fee prior to posting your application

To be eligible for a seniors’ passport you must be at least 75 years of age at the time of application.

Additional Postage Fees
If you require your passport to be returned by Posta Raccomandata you must include an additional payment of €5.00

Embassy Opening Hours
The Embassy is open to the public from 0900 – 1700, Monday to Friday. IMPORTANT – Should any part of your application be incomplete, the form will be returned to you unprocessed. Please call in advance if attending in order to check that we are not closed for public holidays.

Si prega di chiamare il nostro ufficio per fissare un appuntamento se avete intenzione di presentare la domanda di persona.

Raccomandiamo di leggere attentamente le seguenti istruzioni insieme a quelle riportate sul retro del modulo prima di iniziare a compilare lo stesso.

Informazioni Importanti:
E’ obbligatorio compilare le sezioni 1,3,4 e 5 del modulo.
Una volta compilato il modulo deve essere rispedito al nostro ufficio insieme a:
• Il passaporto da rinnovare
• Due fotografie
• La fotocopia di un documento di identità recante foto e firma (per esempio: patente di guida o Carta d’Identità Italiana)
• Pagamento tramite “assegno circolare” intestato a “Australian Embassy” o “Ambasciata d’Australia”.

Indicazioni relative ai costi e alle eventuali spese postali sono evidenziate di seguito.

La preghiamo di:
• Utilizzare esclusivamente una penna NERA
• Scrivere dentro i riquadri in STAMPATELLO
• Rispedire insieme al modulo anche il passaporto da rinnovare. Questo sarà annulato e sarà restituito insieme al nuovo passaporto.
• Firmare solamente ALL’INTERNO del riquadro bianco della sezione 5. Qualora la firma dovesse uscire dal bordo del riquadro bianco dovremo rispedire indietro il modulo e richiedere una nuova firma.
• Le fotografie inviate devono rispettare i parametri evidenziati nel retro del modulo. Qualora le fotografie non dovessero rispettare tali indicazioni, il modulo sarà rispedito e sarà richiesto di allegare nuove fotografie. (Si prega di fare attenzione a non sorridere mostrare i denti)
 

I costi indicati sopra sono soggetti a cambiamento senza preavviso. La preghiamo di contattarci per avere conferma dell’importo prima di spedire la domanda di rinnovo.

Per poter richiedere un passaporto senior bisogna aver compiuto 75 anni al momento della richiesta di rinnovo.

Se si richiede che il nuovo passaporto sia consegnato tramite Posta Raccomandata, bisogna aggiungere l’importo di 5 euro.

Orari di apertura dell’Ambasciata
L’Ambasciata è aperta al pubblico dalle 9,00 alle 17,00 dal Lunedì al Venerdì.
 

IMPORTANTE: Nel caso la richiesta dovesse essere incompleta, il modulo vi sarà restituito senza avviare la pratica.
